Be sure to read the email from Mr. Stull about the vaccines...

PLE will be vaccinated on Wednesday at the GEC.  As soon as I receive a schedule I will get it to you.  

You will need to bring two documents with you to the GEC on Wednesday to get your vaccine.

1. One document requires you to answer questions and to sign. We printed this off for you and put it on the counter in the office. You must have this completed and must take this with you to receive your vaccination. 

2. After the vaccine arrives on Monday, the health department will send us another form you will need to complete. The additional documents provide a HIPPA notice; a Moderna COVID-19 Vaccine information; and an information sheet on v-safe, a smartphone-based tool you could opt to utilize following your vaccination. We will print it for you when we get it.

Teachers-please make sure desks are cleared off before you leave the room for recess or morning gym time. Dawn uses her ghostbusters backpack at that time and she doesn't want students' "things" to get wet.
